Experiencing dysfunction or discomfort around your jaw? From jaw pain to clicking and even headaches and ear pain, TMJ disorders can show up in many different ways. DR MATTHEW WONG from Elements Dental explains what the TMJ is, what causes these symptoms and where to go for TMJ treatment in Singapore.
What exactly is the TMJ and what are some common TMJ disorders?
The temporomandibular joint (TMJ) is the hinge joint connecting your jawbone to your skull, located directly in front of each ear. It enables essential daily movements like chewing, speaking and yawning.
TMJ disorders (TMJD) refer to conditions that cause dysfunction or discomfort in this joint and the surrounding facial muscles. One common condition is myofascial pain syndrome. It involves discomfort or spasms in the muscles controlling jaw movement. Another is internal joint derangement, where the joint’s protective articular disc becomes displaced or structural wear or joint dislocation occurs.
Degenerative joint disease can also affect the TMJ. For example, conditions like osteoarthritis or rheumatoid arthritis can impact the cartilage in the jaw joint.
What are the causes or risk factors for TMJ disorders?
TMJ issues often stem from a combination of mechanical, structural and behavioural factors:
- Bite misalignment (malocclusion or occlusal dysfunction): This is the most common cause of TMJ issues. Uneven contact between the upper and lower teeth forces the jaw muscles and joints to work harder or rest in an unnatural position.
- Teeth grinding and clenching (bruxism): Excessive forces placed on the jaw joints during sleep or high-stress periods.
- Physical trauma or injury: Impact or jaw injury that misaligns the joint or damages the articular disc.
- Arthritis and joint wear: Age-related wear and tear or inflammatory joint conditions.
- Stress and muscle tension: Involuntary jaw clenching triggered by chronic stress.
What are some of the most common signs and symptoms?
TMJ disorders present in a variety of ways. Some people notice joint sounds, like clicking, popping or grating noises when opening or closing the mouth. Others experience localised pain – a dull, persistent ache in front of the ears that can radiate into the jaw, cheek or neck.
Morning discomfort is also common. This shows up as jaw fatigue, temple headaches or a feeling of tightness in the teeth upon waking. Some people find their jaw movement becomes restricted, making it difficult to open the jaw wide, or they experience a jaw that feels “locked”. Over time, excessive bite forces can also lead to uneven tooth wear, including worn enamel or fractured teeth.
How do you diagnose TMJ disorders?
Accurate diagnosis requires distinguishing joint and muscle issues from other dental or sinus conditions. At Elements Dental, we’ll do a clinical evaluation of pain intensity, chronology, quality, localisation and movement triggers. We will also do an occlusal and bite analysis to check how the teeth interlock and evaluate forces across the arches. In addition, we use digital imaging and 3D scanning, including intraoral scanners and digital diagnostics, to map jaw movement and tooth wear patterns.
Is clicking or popping without pain something to be concerned about?
Clicking or popping without pain is very common. It typically indicates a slight displacement of the soft disc inside the joint. While occasional, painless clicking usually does not require immediate invasive treatment, it serves as an indicator that the joint mechanism is not moving seamlessly. Having it assessed during a routine visit helps ensure it doesn’t progress to painful dysfunction or disc damage over time.
What treatment options are available for TMJ disorders at Elements Dental?
Following the Kois Center philosophy, TMJ treatment focuses on conservative, predictable and functional solutions. These are designed to protect long-term tooth and joint health:
- Splint therapy & night guards: Custom bite splints to unload joint pressure, relax jaw muscles and protect teeth against bruxism.
- Orthodontic bite correction: Aligning teeth to correct crossbites, overbites or narrow arches, allowing the lower jaw to close in a harmonious resting position.
- Occlusal adjustments & restoration: Minor tooth adjustments or restorative work to rebuild lost vertical dimension and balance bite forces evenly across all teeth.
- Full-mouth rehabilitation: Comprehensive reconstruction for cases with severe tooth wear or complex bite collapse.
- Referral: If the cause of TMJ pain is determined to be due to pathological problems or deformities in the joint itself, appropriate referral to orthopaedics will be arranged.
- Muscle relaxants or botulinum toxin: TMJ pain caused by subconscious muscle hyperactivity can be symptomatically addressed using muscle relaxants or botulinum toxin as a temporary or transitional solution.
Does TMJ ever fully heal?
With appropriate management, many patients experience complete relief from symptoms and long-term functional stability. While joint damage like severe arthritis cannot always be completely reversed, TMJ disorders can be effectively controlled by addressing root causes. For example, this may include correcting bite misalignments, shielding the joint from clenching forces and restoring harmonious occlusion.
